For Source & Age Approved Ranches:

• Source & Age Enrollment Packet - this packet is necessary to enroll your home-raised calves in the source and age verification program.
• Ranch/Farm Shipping Record - this form must be completed if your calves have moved to a feeding location prior to starting the source and age verification process.
• Tag Disposition Report - this form must be completed and returned to IMI if you have any un-used or destroyed program tags.
• Where Food Comes From^® Supplier Profile - this profile can be used to submit the story of your family and ranch, which will be featured on www.wherefoodcomesfrom.com.

For Non-Hormone, Natural or Grass-Fed Ranches:

• Non-Hormone Treated Cattle Feed Letter Template - this letter must be distributed and signed by all feed manufacturers distributing feed to your location if you plan to be NHTC approved.
• Verified Natural Beef Feed Letter Template - this letter must be distributed and signed by all feed manufacturers distributing feed to your location if you plan to be Verified Natural approved.
• Ranch Shipment Truck Card - this document is to be completed by the ranch when shipping program cattle to the feedyard. This is specific to our onsite ranch custmers approved for SAV, NHTC, Verified Natural, and/or Grass-Fed.
• Tag Order & Age Enrollment form - this form is to be completed and sent in for locations (1)ordering tags, or (2) shipping cattle prior to the next scheduled onsite audit.
• Tag Disposition Report - this form must be completed and returned to IMI if you have any un-used or destroyed program tags.
• Where Food Comes From^® Supplier Profile - this profile can be used to submit the story of your family and ranch, which will be featured on www.wherefoodcomesfrom.com.
